For Inactive Facility:

? Expand Facility Location
? Change Facility Layout
For active facility
? Add/Remove Specialty
? Change Category
? Expand Facility Location
? Add/Remove Add-Ons
? Change Facility Layout
? Update Accreditation and FANAR License ((this amendment does not require payment or inspection)

* Floor plan from engineering/design company
* Trade license (if available)
* Proposal letter (Business plan)
