For the Self-Assessment Tool:

For the Review Registration Eligibility application:
1. Recent passport size photograph
2. Valid passport copy
3. Educational qualification(s)
4. Experience certificate(s)
5. Professional license(s) / registration(s)
6. Good Standing Certificate(s) (valid and not older than six months at the time of application)
7. Last two years logbook (for surgical specialties, if applicable)
